Is it possible to run multiple patches (or instances of the same patch) simultaneously?

Or is PD, and LibPD by extension, really designed as a one-patch-at-a-time system.

If it is possible to run multiple instances of the same patch, is there any way to differentiate which instance I'm sending messages to?
